2. form medical certificate (completed by the occupational medicine physician)
Please make sure that your medical certificate is issued for the entire period of study.
To obtain the required medical certificate, please go to occupational medicine physician with the right to practice the profession in the territory of the Republic of Poland.
The medical certificate must bear the stamp of a physician with the specialization "occupational medicine physician" or, in the case of physicians with a different specialization authorized to carry out preventive examinations, "the preventive examination was carried out by: name and surname of the physician.".

If the doctor issues a medical certificate on a different form, check whether the name is entered correctly. university, address and field of study.
