Certificate of no criminal record and other documents
We kindly inform you that from February 15, 2024. Pursuant to the Act of 28 July 2023 amending the Act - Family and Guardianship Code and certain other acts, in order to be able to carry out student professional internships/clinical classes/practical classes during which there is any form of contact with minors (under 18 years of age), it is necessary to provide certificates of no criminal record.
You can obtain the relevant certificate from the National Criminal Register (KRK) in two ways:
1. Personally – you should report to a convenient Information Point of the National Criminal Register (list of points), which is located at the common courts. The fee for obtaining a certificate is 30 PLN.

Please complete the form with your personal data and:
a) in point 11 please enter: "„admission to activities related to the treatment, education or care of minors in connection with the implementation of the study program”,
b) in point 12 please indicate: "„criminal record”" and "„juvenile records”.
c) in point 13 enter: "„crimes specified in Chapters XIX and XXV of the Penal Code, Article 189a and Article 207 of the Penal Code and the Act of 29 July 2005 on Counteracting Drug Addiction (Journal of Laws of 2023, item 172 and of 2022, item 2600), or for prohibited acts corresponding to these crimes specified in the provisions of foreign law”.
IMPORTANT – The certificate received must be delivered in the original version to the dean's office.
There aren't the possibility of sending the certificate by e-mail.
2. Electronically - you should enter page.
In order to obtain a certificate electronically, you must have a qualified electronic signature, a trusted signature or a personal signature.
The fee for obtaining the certificate is 20 ofł.
IMPORTANT – The received certificate should be sent electronically to dziekanat@wsz.pl in XML and PDF format. A printed certificate cannot be considered a valid document.
Please complete the form with your personal data and:
a) in point 11 please indicate: "„criminal record”" and "„juvenile records„",
b) in point 12, where there is space in the table to enter the scope of data that is to be the subject of information about the person, enter in the table: "„crimes specified in Chapters XIX and XXV of the Penal Code, Article 189a and Article 207 of the Penal Code and the Act of 29 July 2005 on Counteracting Drug Addiction (Journal of Laws of 2023, item 172 and of 2022, item 2600), or for prohibited acts corresponding to these crimes specified in the provisions of foreign law”.

Deadline for submitting documents:
a. By the end of the 1st semester:
– physiotherapy,
– dietitian,
– psychology.
b. By the end of October this year at the latest:
– nursing.
If you fail to provide the required documents, you may not be allowed to participate in professional internships/clinical classes/practical classes.
At the same time, we would like to inform you about the obligation to immediately provide a new (current) certificate in the event of any change.
Moreover, students who have resided in countries other than the Republic of Poland and their country of citizenship (foreigners) over the last 20 years are required to submit a declaration about the country or countries in which they resided, together with information from the criminal records of that country/countries, for the purposes of professional or volunteer activities related to contacts with children.
Criminal record information must be provided with a certified translation.

The declaration is submitted under pain of criminal liability for making a false declaration.
Information from criminal records applicable to foreigners and Polish citizens who have resided in another country/countries in the last 20 years:
1. Must be submitted in the language of the country/countries in question, together with a certified translation.
2. In a situation where a given country does not provide for the preparation of the indicated information or does not maintain a criminal record in the above-mentioned scope, then a declaration should be submitted – A statement that the state will not release information or maintain a register.
*Students whose country of citizenship, for political reasons, might not issue the relevant certificate/information may provide the above declaration.
3. Statutory consequences resulting from failure to submit the required documents and declarations Pursuant to art. 23 sec. 3 of the Act on Counteracting Threats of Sexual Crime, in the wording in force from 15 to 02.2024, a person who, before being admitted to activities related to, among others, education, treatment or care of minors, fails to fulfil the obligation to submit the information or declaration referred to in art. 21 sec. 3-7 of the Act, shall be subject to the penalty of arrest, restriction of liberty or a fine of not less than PLN 1,000.
Legal basis:
Amendments to the Act on Counteracting Threats of Sexual Crime (Journal of Laws of 2023, item 1304, as amended) were introduced by Article 7 of the Act of 28 July 2023 amending the Act - Family and Guardianship Code and certain other acts (Journal of Laws of 2023, item 1606).
